New Client Inquiry
Thank you for your interest in working with BFF Clinic. Please take the time to read over the information below before submitting your inquiry.
All paperwork/intake requirements must be completed within 48 hours of your scheduled appointment. This includes insurance information and the credit card that is placed on file for copays, missed sessions, or late/cancellations fees.
Self-Pay/Out-of-Network Clients: Per our policy, a $100 deposit is required to book your initial evaluation. This deposit will be applied to the total of the initial appointment fee. The deposit is refundable if cancellation is more than 24 hours notice. If you cancel with less than 24 hours notice, the fee is non-refundable. Your deposit must be paid at the time of booking.
